  • I. Information on procedure concerning the shipping of the cremated remains of a deceased person to Poland

    The Consular Unit of the Polish Embassy in Ottawa requires the following documents in order to issue a certificate allowing the entry of the cremated remains to Poland

    • Burial permission issued by local authorities in Poland - office of the starosta or prezydent miasta , such a document is arranged by the family of the deceased person - a copy of the document sent to the Consulate by fax or e-mail is sufficient.
    • Statement of Death, Form 15
    • Medical Certificate of Death, Form 16
    • Certificate of Cremation,
    • Certificate of contents
    • Polish Passport of the Deceased (if available)

      Please, attached also the following information:

    • Personal data of the Deceased (full name, date & place of birth, date & place of death, last address),
    • Name of place from where the cremated remains will be transported to Poland
    • Name and address of the cemetery in Poland
    • Information about mean of transport

    Person responsible for shipping of cremated remains (full name, address, telephone, ID's number.)

    Please provide us with copies of required documents and information by fax (+1 613789 8835) or e-mail ottawa.amb.wk@msz.gov.pl .

    Please note that the Consulate requires the burial permission issued by STAROSTA or PREZYDENT MIASTA. The certificate issued by a priest or a local parish will not be accepted.

    An appropriate consular fee is collected according to the Table of fees.

     

    II. Information on procedure to issue a certificate for shipping of a body to Poland

    The Consular Unit of the Polish Embassy in Ottawa requires following documents in order to issue a certificate for shipping of a body to Poland:

    • burial permission issued by local authorities in Poland (office of the starosta or prezydent miasta); such a document is arranged by the family of the deceased person (a copy of the document sent to the Consulate by fax or e-mail is sufficient),
    • Statement of Death, Form 15 (original),
    • Medical Certificate of Death, Form 16 (original,)
    • Certificate that the Deceased was not afflicted with any communicable disease, issued by Associate Public Health Officer,
    • Certificate for Shipment Body Outside of Ontario issued by Ministry of the Solicitor General, Office of the Chief Coroner,
    • Burial Permit,
    • Certificate of embalming and packaging issued by Public Health Officer.

      Please, attached also the following information:
